Notes:

Copy of report is mailed to League Convener; President, Vice-President Of Operations and Umpire In Chief. The sender will also receive a copy as an audit trail. The sender is asked to check the contents of his/her email to ensure accuracy of information submitted.
Data about sender is gathered and reported within e-mail message in the interests of preventing hoax reporting.
